Point students are responsible for submitting a formal application to the institution for which this permission is granted. It is the student’s responsibility to adhere to all admissions deadlines and requirements.

Take the completed form, along with course descriptions from the accredited college of choice, to the Registrar’s Office for approval. The registrar’s signature is required for official approval. You may also send the form to:
